My car sounds like a boat when starting V70-XC70 2001

This only happens occasionally, but my '01 XC can sound like a diesel boat engine chugging when you start it after it's been sitting a while (train station after work, for example). Sometimes it finds a normal idle in a few seconds, sometimes it does this for 10 seconds or so then the engine dies. When I start it again, it idles normally.

I had the car at a Meineke for oil change and their mechanic asked about it. He had never heard of this but they don't get a lot of Volvos (he admitted).

Has anyone had this happen and knows what to do or what to fix?
