Sounds suspiciously like the head gasket. Check to see if there are green beads of coolant on the end of your dipstick. You can drive the car this way, but not for long without causing damage.

The head will need to be removed to check for cracks (not likely), warpage (possible), or cracked gasket (most likely).

Once the head is off, at a minimum the mating surface will need to be machined. You might consider a valve job, maybe port polishing.
