Oil seperator

I SEEM TO THINK THAT THE oil separator IS pluged and it seems like a bear the replace. Isee that it has 2 ports on it, usually inside one is lower and one is higher. Which hose goes to which? Is it possible to shoot some cleaner down one and vacume out the flush from the other line? Or will this just break up any goop and be pushed or pulled into the engine and cause big problems?
I'm 1st going to check the PTC for dirt, getting a blow-by from the dip stick tube. Would this give the symptom of a slight miss at low to medium throttle on the highway? If this is not addressed will it really blow the rear seal?

P.S. Changed the fuel filter today, had 55k on it and put a bottle of Amsoil fuel cleaner in...made it better. But not gone.
