Nick,

In addition to John O's instruction, before pulling the knob straight up, twists the upper part of the leather boot anti-clockwise to release the boot from the knob.

After tapping the new one in, reconnect the boot back to the knob in clockwise direction. I did mine 2 years ago. You really pull it up HARD to take it out of the shift shaft.
