Something fell off inside driver's door V70-XC70 1998

Noticed a "clunking" upon closing driver's door recently, seemed to be getting worse and today, it's really bad. Then noticed that I couldn't roll down driver's window more than about six inches before it is jammed against something inside the door. I can now hear something clunking upon braking and accelleration, and it appears to be located toward the rear of the driver's door. Popped out the red door light, and stuck my finger inside the door and I can feel something metal that is loose.

Soooo...

1. What is it that has fallen off?

2. How do I get the door panel off to put it back together?

3. Do I need to take it to the dealer? (I have the volvo extended warranty, but I don't know if it would cover this problem).
