Random Rough Idle + Fan noise with exceleration V70-XC70

The saga continues. Volve v70 T5 turbo - 160k.

A week ago, low oil (2.5qts), thought it was bad turbo or VVT solenoid. Car sat for a 3 days, started up and ran smooth. 5 days later, symptoms back. Wife drove car, once warm and after driving for an hour. Same issue back, rough idle, and stalls at stop sign.

Second symptom, on when she accelerates hard, there's a fan like noise that revs with the engine.

Any and all help greatly appreciated.
