At a loss Any suggestions S60 2006

At idle which is around 675 rpms I get a slight vibration in the steering wheel.

The idle is stable doesn't fluctuate i just have this slight vibration. About once a month i throw a p0101 and p0170 which i clear and about a month later I get it again. The car has pep but my gas mileage is only 19mpg

The pcv was replaced 6 months ago, the ptc was cleaned and I have a new MAF Bosch.
I have new motor mount lower passenger and the upper looks fine.
I replaced some of the vacuum lines even though I hooked up a smoker and couldn't detect any leaks.

I scanned the fuel trims. The LT fuel trim is at 0%. The ST only fluctuates +- 3%. at idle and under load.

My fuel pressure is around 44psi.

I have 4 s60's the others are smooth at idle so I know this slight vibration is not right.

Any one experience this problem or have any suggestions.
