Mike,

I have a 1995 850 Turbo with a remote system and it is the side by side button type control. Left button says "LOCK" and right button says "UNLOCK". On the back of the control is the following information:

ALPS ELECTRIC CO. LTD.
DOC/MDC: 1788 K988
MODEL/FCC ID: CWT5653T
P/N 3545653 JAPAN

I have seen these on eBay. Hope this helps.
