Volvo 855 T5 - Burnt valve on cylinder #1 850

This problem has stumped me and my local Volvo dealer. The car has 115k miles on the clock its an 1995 Volvo 850 T5 GLE. The car has been serviced every year, without fail by the same dealer for the 6 years its been registered.
At around 55k miles the engine suffered a instantaneous loss of compression in cylinder #1. When investigated it turned out that exhaust valve #1 had lost a chord of its circle. Bit never found but presumed vapourised. Valves 1 & 2 where replaced.

All has been well for another +55k miles when 10 days ago, the same loss of compression occured in cylinder #1. The head was removed yesterday. As fate would have it, valve #1 has failed again. This time a pie shaped piece is missing. Again the piece lost is no where to be found.

The Volve mechanics have no idea what might be the cause, but believe that something is responsible for the problem. Having such a problem occur once is rare, but twice is too suspicious.

Have any of you experience of such a problem and what might be the cause?
