slipping clutch/grinding noise???

I road tested my new volvo (the $100 model). I adjusted the clutch a little to see what would happen, and actually made some progress, but still nowhere near correct. Here's the case:

When I push the cluch half way in, all gear engage correctly. Too far and the gear grinds (if I can even get it in gear).

When I'm driving down the road, and accelerate more than moderatly, the clutch starts slipping and the RPMs go Way up.

Anybody know what the cause is, and how I can go about fixing it? The Volvo is a 1981 DL 4 cylinder M46 w/OD if I'm not mistaken.

Thanks,
