Inconsistent transmission V70-XC70

My car does more than shift hard. It has a dramatic pause between 2 and 3. This happens inconsistently, usually when I am not driving hard, which is how I usually drive. It happens more when it is cold, but it also happens when the car is warm. So it does not seem to happen at higher RPMs when it is cold, as you indicated it is designed to do.
