front end V70-XC70

I have a 99 V70 with 83,000 miles. I developed a front end vibration at about 35k that the dealer supposedly fixed by replacing the front axel and putting in dampeners. They then forgot to tighted the axel and the axle fell off. Now at 82 k it is still doing it with new tires. It vibrated and shimmies like it needs balancing. Any suggestions or similar incidents?
