transmission funkiness S40-V40 2001

I have an '01 S40 with about 16,000 miles. I've had a funky transmission problem since virtually day 1. I see references to similar problems but I see no solutions posted. The problem is quite unpredictable, but it seems to be related to the adaptive part of the transmission and quick stops or possible stop-and-go traffic. When coming to a stop, the transmission seems to suddenly shift from 3rd to 1st....or something like that. There is a major audible clunk. Then the whole car jerks. It happens fairly regularly.....but not predictably. Seems like after a quick/short stop, this phenomenon will occur shortly thereafter. The dealer has given me the "there are no codes stored" story every time. They also say it's at factory specs.
I also have experienced this "sudden neutral" problem, where when accelerating,
there is a momentary period where the transmission appears to be in "N".

However, the sudden HARD downshift has happened far more often and is far more troublesome to me.....especially since the dealer appears to have no clue whatsoever. And since there seems to be NO WAY that this could possibly be anything but damaging to the transmission.

Anybody have any comments/ideas/solutions?
