coolant light smoke and leak, please

hello--

i had to pull off the freeway tonight, due to the coolant light and smoke... the temp gauge never budged and read normal... i added coolant and water 50/50- about a gallon combined... i only just had the coolant flushed (indie mechanic)so i cannot understand why so low...driving back towards home i didn't get half a mile before the coolant light and more smoke... i dropped the cap earlier, couldn't find it in the dark (not under the car) the first time filling the reserve or i would have left it on loosely... the second time ALL the coolant/water was gone again...


does anyone know what this could be, please?
