Engine/turbo surge in 4th gear? S60 2002

I have noticed recently that accelerating in 4th gear at about 60-75 mph that there is a surge and then lag in power...it is quite noticable i have also noticed the tach at the same time is going up and down about 300 RPM in conjunction with the surges... any ideas?

i dont know if this is the transmission playing tricks on me or if the turbo could be causing this.
