The 443, cat converter efficiency low, is a result of your O2 sensor being slow. Yes, it is about time for a new O2 sensor.
I don't know your skill level, but with the right tools it is not extremely difficult. If you have to lay on the ground, it makes it much more difficult.
You could check any and all vacuum hoses for cracking and age, any leaks will cause the O2 sensor to be off and your gas mileage to degrade.
