Noise from interior fan

I have a noise that comes and goes from the interior fan. Sometimes you can bearly hear it, then sometimes it has a bearing failure noise. It is not consistent and it does not depend on fan speed. I have already had this fan replaced by the dealer at 40k. This car is out of warranty. Is there a known problem with these fans, recall etc. Does anyone know if there is a replacement other than the OEM fan.
