O2 Sensor CKT Slow Response (Bank 1 Sensor 1)

That is a direct result of engine management comparing o2s1 with o2s2. You are probably running too rich as a result of a vacuum leak, and the enriched mixture is coating the o2 sensor.

Check for leaks all around the intake manifold. By the way, the reading you quoted from the o2s1 are perfect.
