There is a youtube out there describing the procedure. To get the outer boot off, the inner boot must also come off. Yes, grease does come with the boot 'package' as well as clamps.

It is a messy procedure...

The CV joints will last a very long time so long as they are not exposed to gritty dirt or water. I suspect most of the 'remanned' axles are just re-booted and cleaned up.
