Thanks for ideas, Chris. The "lurch" or "stutter" was fixed by the following:
- new plug and coils
- new air filter
- removed & cleaned throttle body

Lesson 1: not all engine misfires are reported with a DTC
Lesson 2: follow the PM advice to replace plugs and coils after 100,000 miles (I made it to 120,000 before first symptoms of trouble).
