the answer to your first question is that if you have enough money you can do whatever you want to. If you want to replace your entire front dash then, yes, you can order one made for the Dolvy Surround Sound, where you can install the extra center speaker along with the upgraded amplifier.
As far as your 2nd question is concerned, you can check around at different stores where they sell audio equipment and I am sure that you will find a compatible 6-disc changer. Check with your local Volvo Retailer as the prices on the "old" 6-disc changers have dropped dramatically.
Lastly, to asnwer your 3rd question "why Volvo did not install the Dolby feature on all cars that could get the SC-900" it is a matter of cost, both to Volvo and the consumer. Adding Dolby surround sound, would increase the cost by at least another $800. The SC-900 is a huge improvement (both in functionality and quality) over the mainstream stereo system (SC-816) that was availabel in most North American-bound models...
