Sometimes the engine is louder at idle than other times.

It's more likely you are hearing the transmission (assuming its an auto). Put in park and then back in drive and you'll hear the difference. It only gets louder as time goes on. I flushed mine no help. Then I totaled the car (thats a whole nother story, lets just say I could make a volvo crash commercial) so I never found out what was happening.
