Do you drive the car while holding the shifter? I havent seen that in any Volvo MT but in other cars it'd happen if the owner drove holding the shifter alot and that would then wear the hubs shift sleeve for the two related gears and/or the shift fork inside the trans. Fixing it requires a trans overhaul or replacement trans, no simple adjustment.
