Yep. Let's see:

1. Break vibration shakes entire car, not just break peddle.
2. Vibration only happens when breaks are warm.

It sounds like you need to replace the rear shims with the cutout shims. I went ahead and replaced the pads at the same time. I also used the silicon lube between pads/shims/calipers. Problem when away for me and never returned.
