1. I change mine yearly (~20,000 miles) It's cheap (less than $20) and easy

2. Yes, I suggest a new hose retainer (6842411-8; $3.47) for the tranny line

3. www.volvospeed.com repairs & maintainence>Bay 13 has great info/instructions, as does the Haynes manual for 850's (I recommend this to all 850 owners)
