I'm north of 154,000 miles and no, not YET, although I think it did slip once when I was pulling out of a toll booth a couple of weeks ago.
It's not cheap since the engine and transmission both need to be removed. Figure on the better part of a day's labor from an experenced Volvo tech. I'd have to guess $750-$1200 (US) depending on the going labor rate wherever you are.
It's also suggested to have the main seal replaced while everything is apart.
