Alright, more thinking and less posting Will...

A car with a faulty cam sensor wouldn't start. An intermittent fault *should* trip a code.

Sticking with the timing theme, what else could it be? Skipped teeth on the belt from a funky tensioner? How much misalignment would it take?

Oh, for the simplicity of a 240's problems.
