The front glass is a little easier- you need to remove the door panel, then you'll have to remove the glass in the track from the scissor assembly. Don't try to remove the glass from the metal track. This is going to be nasty and you may end up with a lot of broken glass in your door.
It has been a long while, but I wrote up exactly how to do it in a previous post of mine.
Just to make sure - you mean the small piece of glass IN the rear door, not just behind it, correct?
For the rear... I think you also have to remove the rear main window glass.
Once the rear main window glass is out, you need to remove the metal vertical bar that hold the glass in place. The screw and the metal are about the only thing that holds the glass in place. Then, there is an easy-out rivet that you have to drill out (and replace with a sheet metal or wood screw). Be careful when re-installing rubber mouldings, as it's easy to stretch them and wind up with them scrunched up or in the wrong spot.
