No idle, putt, putt 140-160 1975

The Grand Dame ran great for two weeks now. The new owner called me today because she couldn't get the gas cap off. Several strangers also couldn't get it off. It seems there used to be a key for it and it locked itself! The key to unlock the doors fits but doesn't do anything. Are there a set number of keys for the cap or will I have to call a locksmith?

Now, for the putt, putt. As the car was being driven over to my house, it decided that it didn't want to idle anymore. It runs OK at 2,000+ rpm, but anything less than that and cylinder #1 stops firing.
Closer examination has a bad sounding air leak at #1 intake/exhaust manifold. This means I need to take the heat shield off to expose the bolts. Will a leak cause this problem? Or am I looking at the wrong place? It has a newer distributer, cap/rotor/wires/plugs, etc.
