1993 Volvo 240 Sedan: Engine shakes when cold 200 1993

My wife owns a 1993 240 Sedan with only 97k miles. Recently, on occasion, when starting the vehicle cold, the engine shakes a great deal. I installed new plugs and we have added Techron to the fuel supply to ensure that the fuel injectors remain unclogged. We are uncertain what causes the 'engine shake' other than a possible intermittent malfunction of a fuel injector. Can anyone lend some advice?
