1-1-3 code with LH3.1 200

Could a faulty O2 sensor cause a 1-1-3 code on LH3.1 without causing a code 2-1-2 (faulty signal from O2)?

The car runs rich after replacing numerous parts. Its been getting better, but still not there yet. Also, where is a ground connections at ECU? I figure it wouldn't hurt cleaning them up. I already cleaned the ground strap on the valve cover and it didnt help.
One more thing, is it safe to clean the o2 sensor with carb cleaner, or would that pretty much kill it?? It has a lot of soot on it from the rich running condition and wonder if that is throwing things off.
