I picked up a 1990 16v 745 from a person who was increasingly overwhelmed and unable to work on cars.. but the car has a smattering of issues;

what i've done so far,
*changed plugs- the old ones were FRIED (not much left of the element) more than I've ever seen,
*filter
*throttle body cleaned
*iac valve cleaned
*vacuum hoses redone except for down tube/flame/oil trap everything
*timing belt

I'm having some issues.

1. When I start it up cold, the car immediately wants to die, and I have to keep my foot on the pedal until it's warmed up. When I put it in drive on a cold start, the idle will rise(and thus the speed of the car) and the car will SURGE forward for about 3 seconds... and then slow down, and it will keep doing this for about 4 minutes until the car is warmed up. Any thoughts on this?

2.. On the outside of the airbox on the lid is what seems to be about an inch of plastic tube sticking out that looks like it needs a hose.. where would this hose connect to on the other side? should I block the hole?

3. I have a really loud whistling noise whenever I let off the gas when driving. it lasts for a few seconds. It sounds like air coming out of the end of a pinched balloon. If I disconnect the MAF, the idle shoots up and down and on the DOWN, you can really hear the whistle.

4. Really really bad smell of gas in the car- and really really bad gas mileage- Like 16 mpg. I know the exhaust manifold is busted and in need of replacement... I'm guessing this is the cause?

5. I'm getting code 2-2-1. Bosch ignition.. I think?
