Turbo Seizure: Rebuild or Replace? 700 91

My turbo has seized. For what reason, I'm not sure, but the turbine is stuck badly. I need a rebuild or replace, I'm assuming. Where's a good place to get a rebuild? Can I do the rebuild myself, or is it quite difficult?

What could've caused the seisure? I had enough oil in the car, I always check it......
