Trouble removing top starter bolt...

Hello, I have read some posts about removing the starter from 240 Volvo's and have a few questions...
I have determined that my Volvo 240 DL wagons starter is bad, so I bought one today at the auto store. I did a bench test on the new starter to make sure that it works and it did. I attempted to remove the lower starter bolt from my Volvo with no luck and decided that I needed a air ratchet to get the bolt off. It worked after about 20 minutes of fighting. Now here's were I need some help... I can't get to the top starter bolt, I have tried using about 20 inches of 3/8 extensions and a flex connected to the 18 mm socket, but I can't get a good grip on the bolt. It keeps slipping. I thought the starter bolts were 19mm from a previous post. But both of my bolts are 18mm.
If someone can please tell me about a special tool or a way of getting the top bolt off I would be sooooo happy. I'm worried that once I get the top bolt off and replace the starter, how will I put the top bolt back in.
Is there a way of getting to the top bolt from the engine compartment?
