Sporatic Idle 700

At times when I first start my car, it will jump from 500 to 1500 rpm and back instantly. Makes for an interesting time pulling out of parking lots and what not... I noticed also that when this was happening my vacuum guage would really spike and drop. So the car seems as though it will stall then the vacuum guage goes way up (IAC opens wide?) and rpm goes to 1500 and will drop again until an almost stall and we start over again....
My questions is does this sound like something giving a bad reading to the IAC motor or should I try something as simple as a IAC motor cleaning? Thoughts?
