AT shift mechanism 200 1988

The shifter in for my automatic transmission is getting a little funky. It seems to sometimes jump a notch, so that when it says drive on the shifter, it's actually in 2nd, and when it says neutral, it's in drive. When it says it's in park, the car is actually in reverse, and sometimes I can't get it to slip into park. This is an intermittent problem. The tranny has about 50K on a rebuild, so I'm pretty sure that nothing's wrong with it. What is the likely cause and/or fix?
