Noisy shift lever AT 240 1990 200

I posted this in the RWD forum. I am re-posting it here in hopes of getting more replies:

My 1990 240, 120K miles, AT just started a new behavior. Upon moving the shifter lever, there is now a new noise, almost like a loud ratchet. I took the internal cover off, there is no obvious problem. Any suggestions as to what this is and how to fix it would be appreciated. If I ignore it, what may happen and how fast?
