1991 240 Wagon Running Poorly 200 1991

I have a 1991 240 wagon with AT and 258,000 miles. Recently it started running poorly on startup. When I start the car, the check engine light remains on and I have to give it gas to keep it running. When I shift to reverse or drive, the car cuts out almost immediatly the first go around. On successive attempts it will run and once warmed up, it will run without keeping my foot on the gas. The diagnostic says the car is compesating for a lean mix. I have changed the fuel filter, Tunned the car and performed initial diagnostics from the unit mounted in the engine compartment. All this to no avail. Can someone provide direction on what might be the fix or other diagnostic checks to perform? Thanks!!
