May a lose TB tensioner do this? ***PICTURE*** 900

Good evening forum,
I'll appreciate if you could share some of your expertise on this accident.

I had replaced the timing belt and tensioner about 3000mi ago. I never re-adjusted the tensioner after installing the belt so this evening I decided to pop the rubber inspection plug and do the untight-retight procedure and to my surprise the tensioner nut was lose to the point that it could be removed by hand.

I also noticed some black rubber dust on the plug I removed to gain access to the tensioner nut, so I decided to open the belt cover and check the origin of the dust.

I found the belt (as you can see from the picture) about 1/4" "overboard" from the camshaft pulley and tensioner. I'm glad I checked!!

MY QUESTION IS: Can a lose tensioner nut cause all this? It might be possible that the nut was never tighten after the belt installation.
