1989 B230F cam belt off one tooth 200 1989

Hello all,

Twice in the last few years I've had my car into two separate mechanics to diagnose a check engine light (1-1-3 code, "fault in fuel injector(s)") and have had the two shops tell me my cam belt was off one tooth, in addition to a few other items (fuel pressure regulator, 02 sensor). Can the belt jump one tooth on the cam that easily? Here are the possibilities as I see it:

1) The first time, a few years ago, the belt was really off one tooth, and the mechanic put it right. Then later, the same belt jumped a tooth again, and my new mechanic found it and fixed it again.

2) The first time, the mechanic just THOUGHT it was off one tooth, and adjusted it to what he thought was correct, but was actually wrong, and so it was then REALLY off one tooth, and the new mechanic found that and put it back where it was supposed to be.

What do you think? If it is jumping teeth all the time, why would it do this, and how do I keep it from happening again? Is the tensioner bad? How big of a fix is that?
