When does a fault, set a code? 900 1996

I have a 96 960 wagon,109,000 miles, and I've been having trouble with a code, P0410. I thoght I had it fixed but apparently not. After cleadring the code I might drive 300 miles before it reappears or sometimes only 120 miles.
My question is does the check enginge light come on at the moment something goes wrong or does the problem have to occur a certain number of times before its tripped?
