Next time it happens at idle, try to determine of there is a rhythm to the unevenness. If so, then a pumped-up tappet or fouled spark plug are likely. 15MPH is too high to be IAC / throttle body (deposit) caused.

If the shake feels more random, a stuck EGR valve is possible if your car has California em package.
