Do you think it might be "dead" spot on the armature of the motor. Since it stops in the same place every time when it is all the way up, it might be that the spot it stops in is marginal in making contact.

Perhaps you can remove the motor and give it a random 12v feed and reinstall it. Chances of the same spot being the stopping spot seem remote and that might cure the problem?
