A question about the T belt tensioner 700 1989

I am pretty sure that I am going to have to replace the tensioner on my B230t, but I was wondering why it even fails. It is nothing more than a wheel on a bearing - with a spring to hold it tight.

The question is - does the spring get to a point that it will not hold the pressure? That seems to be what I have - as the bearing and wheel seem fine. I adjust it, and it seems to work for a short spell - and then I hear the belt trying to flap in there again.

Or maybe I am not adjusting it right? All comments welcome.
