Jerky acceleration 200 1987

1987 24 Dl, M46 5 speed transmission, 154,000 mi: Son is moving to Portland, OR from Eugene and reports jerky acceleration (1000-2200 rpm) intermittant but getting worse. It seems to be related to gas pedal pressure in lower gears as he goes off from a stop, and is felt in the rear of the car. Also reports it to be load dependent(heavy load of stuff in truck). I suspect the in-tank fuel pump, outside pump replaced several years ago. How about a drive line issue?
