Bad rebuilt alternator? - Have voltage at exciter terminal but no charge

Hi All,
Per some previous posts I'm testing this rebuilt alternator which seems to have died after just 3 weeks. Took it back to Autozone and it tested "good" on their computerized thingy but here in this basics world of a $10 VOM it doesn't seem like it's working. I've got no battery warning light (bulb good) and 12v at the small alt. terminal but when started the voltage out of the large alt. terminal never rises above 12v. I disconnected the small terminal and ran a wire directly to the battery with a 1.2w bulb in line and the bulb lights with the car off but never goes off when car is running and still with this direct power to the exciter terminal the voltage never goes above 12. Bad alternator? I have no more ideas. Tell me what you think. Of course I cleaned the grounds and both ends of the large wire to the alt.
