First thought is to have you try Lucid's suggestion or Kyle's advice to diagnose the electrical portion of the system. The "missing 4th gear" on Volvo automatics is no rare event and the causes are quite well understood by the many who have decided to fix the issue. A search of this list will give you an hour's reading. Or, you could check out a diagnostic tip pictorial here.

On the other hand, the electrical complexity has driven many to bypass the OD lockout feature - even IPD sells a solenoid bypass plate.
