Failed Emissions Test 92 240 200 1992

Hi, I have a 92 Volvo 240 automatic sedan with 176,000 km that failed emissions test. It passed the curb idle test:
limit reading
HC ppm 200 51
CO% 1.00 0.09
NO ppm N/A N/A

But failed the ASM 2525 test:
limit reading
HC ppm 62 108
CO% 0.35 0.63
NO ppm 467 2904

I had this same problem a year ago, I gave it a tune up (spark plugs, cap, rotor), changed oxygen sensor (BOSCH 13913), catalytic converter, air filter, removed flapper in air filter box, no check engine light, no codes. Has anyone experienced this before and tell me what it could be? Any help is greatly appreciated. Thank you.
