timing belt fell off? WHY ??? 200

Just replaced my timing belt and tensioner, and put an a "A" cam. All was well, and the car was running real well with a 2.5" cat back exhaust. Felt a significant increase in "pull" with the new cam. But that is not why I am writing.

Was driving today and my car died. Checked a few things and noticed that the camshaft was not spinning. Had the car towed home (ouch!), and discovered that my timing belt fell off the intermediate gear. How common is this? I did retighten the belt after running it awhile initially. The book states to retighten the belt after 600 miles, and it has been about 500 miles, could that be it?
