Here's a shot of the tensioner. Sorry it's not any more clear.

Yes, the block with threaded hole goes at top.
Through bolt anchoring to alternator is in the lower position.
Hex nut tightens against end of through bolt towards radiator.

I'd make the nut hand tight while adjusting belts, then snug it down when done.
